To get ready for your initial 60-minute virtual Zoom appointment, please complete the simple onboarding checklist below. I have triggered emails to you from two separate systems.

Verify Insurance & Billing (Headway)

Check your email for a link from Headway. Headway is a secure, third-party system I use exclusively to bill your insurance.

Entering your information here will instantly verify whether I am in-network for your plan, and it will give you a clear breakdown of your expected copayment or out-of-pocket costs.

Note: If you are paying out-of-pocket and not using insurance, you can skip this step.

Set Up Your Medical Record (Osmind)

Check your email for messages from Osmind. Osmind is our electronic health record (EHR) system. This is where your private medical records live, how you will safely communicate with me, and the system I use to order medications.

You should have received three separate items to complete in this system:

  • Practice Policies & Privacy Practices: You will get an email asking for your signature. Once you electronically sign these forms, they will save to your Osmind account as a PDF for your records.You can review the process via the attached file below if you need further assistance.
  • Intake Form: A link to securely provide your medical history and background.
  • Symptom Scales: Brief questionnaires that measure mood, anxiety, and other symptoms. These help me understand the exact nature and severity of what you are experiencing so we can build the best treatment plan.

Let Me Know You're Done!

Once you are satisfied with the Headway cost breakdown and have finished all the Osmind paperwork, please reply to my original email to let me know you have completed these steps.

As soon as I hear from you, I will reach back out with several date and time options for our initial appointment.
