How much would it cost me?

In-network Insurance? 

If your plan is in-network, then your cost is usually your copay. If you have a high deductible plan, then you may have to meet the deductible before your insurance starts to pay. We can let you know the specifics prior to our initial appointment. You can also check this via the Headway portal. Please complete the form below to initiate the process. 

 

Out-of-Network Insurance or Out-of-pocket? 

Professional Fees

You are responsible for payment at the time of your session unless prior arrangements have been made. Payment may be made by credit card, debit card, or HSA which will be kept on file and charged automatically via the Headway portal. 

 

Rates: For in-network patients, your cost will be  your copay (or any deductibles). For out-of-pocket care, the rates are as follow: 

  • Initial appointment: $350/visit
  • Follow-Up 30 minute appointment: $250/visit
  • Follow-Up 60 minute appointment: $350/visit 

 

Fee Schedule

No-Surprise Act: The No Surprise Act aims to limit the amount you pay out of pocket to a level closer to what you would pay if the healthcare provider were in-network. Frequency of visits:

For medication management, typical visits often vary between monthly to once every 3 months. During the initial phase of treatment, you may be recommended to be seen after a few weeks only as clinically indicated.

Good faith estimates are provided below. Please use the calculator to estimate your annual costs. 

 

Attendance, Punctuality, No-Show and Late-Cancelation Policy: Please attend scheduled appointments on time and notify our office in advance if you need to reschedule.


• All appointments must be canceled or rescheduled a minimum of 24 hours before the appointment. Less than 24-hour cancelation is considered “late-cancelation” and a missed appointment is considered a “no-show”. For both late-cancelation and no-show, you will be responsible for paying the full fee that insurance would pay for that session or a minimum of $150, whichever is higher. Please note that these are out-of-pocket expenses and the patient is not reimbursed by their insurance plans for late-cancelations and no-shows.


• You are responsible for coming to your session on time; if you are more than 15 minutes late, we will not be able to bill for your session and thus, it will be considered a missed appointment (no-show).

How do I use my Out-Of-Network (OON) benefits if my plan is not in-network?

Through Thrizer, you can easily submit your out-of-network claims, often receiving reimbursements directly into your bank account. This simplifies the process, reducing the financial burden and allowing you to focus on your mental well-being.

Do you do disability evaluations or FMLA paperwork?

HolistiCare, LLC – Disability Evaluation & Documentation Policy

At HolistiCare, LLC, our primary mission is to provide high-quality psychiatric care focused on treatment, recovery, and functional improvement. The following policy outlines expectations for patients requesting disability evaluations, work-related documentation, or completion of forms.

1. Scope of Practice
HolistiCare is a clinical treatment practice. Our role is to diagnose, treat, and support recovery from psychiatric conditions. While we may complete disability-related forms as part of ongoing care, we do not function as an independent disability evaluation service.

2. Independent Disability Evaluations
Patients seeking disability evaluations solely for administrative, legal, or benefits purposes (e.g., Social Security Disability, legal cases, independent medical opinions) should seek an independent evaluator in the community.
HolistiCare does not provide one-time or “single visit” disability evaluations.

3. Requirement for Ongoing Treatment Relationship
Most disability determinations require longitudinal clinical knowledge, including response to treatment and functional status over time.
As such:

  • Disability documentation is generally only completed for established patients engaged in ongoing care.
  • A single visit is not sufficient to establish disability status in this practice.
  • The duration and frequency of care required prior to completing forms will vary based on clinical complexity and judgment.

4. Form Completion Process

  • Most disability, FMLA, or workplace accommodation forms require a dedicated appointment.
  • Completion of forms is based on clinical documentation, observed functioning, and treatment history.
  • Fees may apply for form completion and administrative services, depending on complexity and time required.
  • Turnaround times will be communicated at the time of request.

5. Nature of Disability Determinations

  • Our role is to provide objective clinical information, including diagnoses, symptoms, treatment history, and functional limitations.
  • We do not determine eligibility for disability benefits. Final decisions are made by the requesting agency (e.g., employer, insurer, or government entity).
  • Documentation will reflect clinical findings and may not align with a patient’s expectations or desired outcome.

6. Approach to Work Capacity and FMLA

  • When patients are unable to work due to psychiatric symptoms, our goal is to support recovery and return to functional capacity.
  • Short-term work restrictions or leave (including FMLA) may be supported when clinically appropriate.
  • In general, support for work absence is time-limited and typically does not exceed 90 days, though duration may be shorter based on clinical factors.

7. Higher Level of Care Requirements
If a patient remains unable to work due to psychiatric symptoms beyond a short-term period:

  • Continued certification of disability will generally require engagement in a higher level of care, such as:
    • Intensive Outpatient Program (IOP), or
    • Partial Hospitalization Program (PHP)
  • This ensures appropriate treatment intensity and supports functional recovery.

8. Focus on Functional Recovery
Our treatment approach emphasizes restoration of function and return to meaningful activities, including work when possible. Disability is assessed based on functional limitations, not diagnosis alone.

9. Patient Responsibilities
Patients requesting documentation are expected to:

  • Engage consistently in recommended treatment
  • Attend scheduled appointments
  • Participate in treatment planning and higher levels of care when indicated
  • Provide requested forms and relevant information in a timely manner

10. Right to Decline Documentation
HolistiCare reserves the right to decline completion of disability or administrative forms when:

  • There is insufficient clinical information
  • The patient is not engaged in ongoing care
  • The request falls outside the scope of clinical practice
  • The documentation requested is not clinically supported

11. Transparency and Communication
We aim to be clear and consistent in our documentation practices. Questions about this policy can be addressed during appointments.


Patients seeking disability evaluations without the intention of engaging in ongoing psychiatric treatment are encouraged to locate independent medical evaluators in their community.

step 1. contact information

Complete this form and await for a response from us before proceeding to Step 2. Please allow 1 to 2 business days for a response. 

If you are suicidal and either at risk for harming yourself or others, please call 911 or report to the nearest emergency department. 

 












step 2. insurance verification via headway

We use Headway to verify insurance, file claims, and charge any co-payments. 

 

In this step, you will receive an invitation to create your own portal for Headway. Look for an email invitation from Headway (and check spam folder if needed). Please complete your profile on Headway.co as soon as possible. 

 

If you plan to pay out-of-pocket without any insurance plans, you will still use Headway to make payments. 

step 3. register for patient portal via osmind

We use an Electronic Health Record (EHR) system called Osmind

Once your insurance information is verified and you've created your Headway portal in Step 2, you will be sent a link to register for Osmind. 

Osmind is specifically designed for psychiatrist and we will use this portal and associated app. for communications, and maintaining your medical records. 

Please set aside 10 to 15 minutes and provide as much detailed background information as possible. The greater the details, the better understanding we will have of your medical/psychiatric history, and an optimal diagnosis.

 

Don't forget to download the Osmind app. which will allow you to message us via this HIPAA-compliant, secured app. 

 

step 4. appointment confirmation

Once you are onboarded in Osmind (Step 3), then we are able to communicate with you regarding times and dates for an initial (intake) appointment. 

 

You will receive a text and email with the date/time and a secured, HIPAA-compliant Zoom link.
